Correction of Student Records

Student Record – refers to the student’s information submitted to Registrar’s Office as enrollment requirement.

Procedure on Updating Student Records (Major Correction)
Major correction would cover updating of students record for:

  1. Name of student
  2. Birthdate
  3. Gender

Requests RO to update the submitted student records and attached supporting documents Filled-out Request Form with attached documents Student
Validates the request and the attached supporting documents Validated Request Form and attached documents Registrar's Office
Instructs the student to process updating of records with government agencies Endorsement Letter with attached supporting documents Registrar's Office
Process to government agency to process correction of records:
1. High School Students to DepEd Regional Office
2. College Students to CHED Regional Office
Submitted endorsement letter to the Records office of Government agency Student
Informs RO to update the student's records Endorsement Letter addressed to Mapúa MCM Government Agency
Submits the endorsement letter from government agency to RO Received Endorsement Letter from government agency Student
Implements the updating of student records Updated RO Records Registrar's Office
