Document Requisition

Allows students and alumni to request official academic documents such as transcripts of records, student permanent records, certificates such as enrollment, honors, ranking, grades, diplomas, and other scholastic academic records. The office also handles record verification/academic record updates.

For document requests, requestors may visit the Registrar’s office from Mondays to Fridays, between 8:00 AM to 5:00 PM, and on Saturdays from 8:00 AM to 12:00 PM, except on declared holidays. The processing time for document requests is 7 working days upon confirmation of payment.

Please note that student clearance is required as part of the processing of student credentials. If not cleared yet, the requested documents will not be processed.

We highly encourage that students should submit their requests in advance, as we receive a high volume of requests. Same-day release of documents is highly discouraged.

Common documents being requested:

  1. Transcript of Records/TOR (evaluation/informative, board exam, official, etc) - An official document only applicable to college students/alumni containing the student’s complete academic records, subjects taken, and grades earned.
  2. Certificate of Transfer Credential/Honorable Dismissal – A scholastic record issued to college students only when transferring to another school, certifying that they are officially cleared and permitted to transfer.
  3. SF10/Student Permanent Record (evaluation/official) - A scholastic record of a student’s academic history used for school evaluation, transfer, or enrollment purposes. Formerly known as F137.
  4. SF9 / Report Card (CTC/official) - An official document showing the student’s academic performance and grades for a specific academic period or school year. Formerly known as F138.
  5. Certificates (enrollment, honors and awards, ranking, latin honor, course description etc) -An official statement issued by the Registrar certifying information.
  6. Certified True Copy (grades, GSA, etc.) - A verified photocopy of an original school document authenticated by the Registrar.
  7. Diploma - An official document awarded to students upon completion of their academic program, serving as proof of graduation and degree completion.

Those who can request/claim the documents:

  1. Student/Alumni
  2. Representative – given that they can present an authorization letter coming from the owner of the document to be requested or to be claimed. (Complete name of the student, representative, list of documents to be requested/claim/transactions to be processed, a copy of the ID of the student with three specimen signatures and a physical copy of the representative’s ID.)
  3. The requesting school – must present an official request letter of the permanent record or official transcript. Or must present the bottom half of the honorable dismissal with the official name and signature of the school’s registrar/director of institution.

How to request the documents for Transfer Out Credentials (transferring out students/alumni):

  1. Go to the registrar and undergo clearance signing.
  2. The student must be cleared from the office of the Registrar (no lacking documents), Treasury (no outstanding balances), and from the OSS.
  3. Students not cleared from the 3 offices will NOT be allowed to go through with their requisition. They must first complete their lacking documents from the registrar and settle any remaining balance at the treasury before being allowed to request the transfer-out documents.
  4. Students that are cleared will be billed for the requested document and must pay at the treasury office within the day (delay in payment will result in delay of the processing and claiming date).
  5. Then the document will be processed within 7 working days upon confirmation of payment.
  6. Upon claiming, the student/representative (with authorization letter) must present their valid ID along with the claim slip.

How to request the documents for Request Credentials (enrolled students in the current term/sem):

  1. Go to the registrar
  2. The student must be officially enrolled in the current term/sem to be able to request.
  3. When confirmed, the student will be billed for the requested document and must pay at the treasury office within the day (delay in payment will result in delay of the processing and claiming date).
  4. Then the document will be processed within 7 working days upon confirmation of payment.
  5. Upon claiming, the student/representative (with authorization letter) must present their valid ID along with the claim slip.

Delivery/Releasing Method

  • Mail (if it’s a school-to-school transaction)
  • Hand carried by the student/representative
